Long Hair FUE: transplanting without shaving
For decades, the standard FUE hair transplant protocol required completely shaving the donor zone and, in many cases, also the recipient area. For most patients, this meant weeks of visible evidence of having undergone surgery — an emotional and social price many were unwilling to pay.
The Long Hair FUE technique changes that radically.
What is Long Hair FUE
Long Hair FUE is a technical variant of FUE hair transplantation in which follicles are extracted and implanted while maintaining the hair at its full length. The result: the patient can return to their environment 24-48 hours after surgery with the result already visible — what Dr. Trivellini calls the "preview": an immediate preview of the final appearance.
It's not an aesthetic trick. It's a complete surgical procedure with the same long-term results as conventional FUE, but with a radically different postoperative experience.
The technique's history
The original idea of working with long hair in hair transplantation is attributed to Brazilian surgeon Marcelo Pitchon, who developed the initial "preview" concept. But it was Dr. Luis Roberto Trivellini who systematized the technique, perfected it, and — crucially — invented the specific surgical instruments needed to execute it consistently with high graft survival rates.
Both received the ISHRS 2021 Platinum Follicle Award for their joint work on this innovation.
The technical challenge
Working with long hair during follicle extraction and implantation is significantly more complex than conventional FUE:
Extraction:
- The long hair shaft easily tangles with conventional circular punches
- Traction on the shaft can damage or sever the follicle during extraction
- Work speed is lower: the 800-1,200 grafts/hour of conventional FUE cannot be achieved
Implantation:
- Long hair obstructs visualization of microincisions
- The implanter must guide the graft without bending or tangling the hair
- Greater coordination is required between surgeon and team

The tools that make it possible
Dr. Trivellini designed specific instruments to solve these problems:
Ring Punch (octagonal punch) A punch with 8 perimetric indentations that protect the hair shaft during instrument rotation. This eliminates the main cause of transection in long hair extraction. Published in the Journal of Cosmetic Dermatology (2021), this punch achieves over 95% intact long-hair grafts — comparable to conventional short-hair FUE rates.
Trivellini Long Hair System The version of his motorized FUE device specifically adapted for this technique. Includes a pre-configured program with optimal parameters of speed, movement mode, and suction for long hair.
T-Fast Multi-Implanter An implanter with two needles that allows implanting two grafts simultaneously. By having multiple loaded implanters, the ischemia time (time outside the body) of each graft is drastically reduced — critical in longer procedures.
The "preview" as a conversion tool
Beyond the clinical aspect, Long Hair FUE has a direct impact on patient experience and clinic conversion rates:
-
Eliminating the shaving barrier: many patients — especially women and men with long hair — ruled out transplants to avoid shaving. With Long Hair FUE, that barrier disappears.
-
Visible result from day 1: the patient immediately sees their appearance with the new density. This generates confidence and satisfaction from the first moment.
-
Postoperative discretion: 48 hours after surgery, the patient can return to normal life without it being evident they had surgery.
-
Justified premium: technical complexity and lower graft volume per session allows clinics to charge a price differential vs conventional FUE.
Who can perform Long Hair FUE
The technique requires:
- Prior mastery of conventional FUE (minimum 500-1,000 procedures)
- Specific training on Trivellini Long Hair instruments
- Surgical team trained in long hair management during implantation
- Adjusted productivity expectations: 800-1,500 grafts per session instead of 2,000-3,000 for conventional FUE
This learning curve explains why, despite existing since 2018-2019, very few clinics worldwide offer Long Hair FUE with quality guarantees.
Conclusion
Long Hair FUE isn't for every clinic. But for those investing in proper training and the right instruments, it opens a market segment — discreet patients, women, professionals who can't "afford" visible recovery time — that conventional FUE cannot reach.
