Return on investment of hair clinic software: the maths with 20 surgeries a month

Die Blogartikel sind auf Spanisch und Englisch verfügbar. Wir zeigen sie auf Englisch an, weil es keine Version in Ihrer Sprache gibt.

Before buying all-in-one software, the sensible question is not how many features it has but how much a clinic has to improve for the plan to pay for itself. This article does that sum with round numbers and the assumptions in plain sight, so you can swap them for your own. These are illustrative figures, not a measurement of any clinic.

The starting point

A hair transplant clinic doing 20 surgeries a month at an average of EUR 3,000 per surgery bills around EUR 60,000 a month, EUR 720,000 a year. It is a prudent example price: the market references cited by the ISHRS are higher, around USD 4,900 to 5,800 on average in Spain and around USD 12,500 to 13,500 in the United States. At EUR 8,000 to 10,000 per surgery, the same schedule would bill between EUR 160,000 and 200,000 a month.

What the software costs

Plan prices are changing, so we do not fix them here: always check the pricing page. For the maths, use three reference monthly fees (excluding VAT): EUR 500, 1,000 and 2,000. Against the example billing, they are 0.8%, 1.7% and 3.3% of what the clinic bills in a year.

One more surgery

The most honest way to work out the return is to ask how many additional surgeries have to come in to pay for the software. Using revenue (not profit) of EUR 3,000 per surgery:

Put differently: a single extra surgery a month covers the fee several times over, even at prudent surgery prices. If you would rather measure against profit than revenue, divide the fee by what you keep from each surgery (at a 40% margin a EUR 3,000 surgery leaves EUR 1,200, and a EUR 1,000 fee is covered by 0.8 surgeries a month). The 40% is an assumption for illustration; use your own.

Where that surgery and those hours can come from

Software does not operate and does not persuade anyone. What it can do is remove friction in four places, and each has a number you can measure this week:

1. Calls and messages nobody answers. Count how many calls the front desk misses after hours or with a full schedule. The voice agent and the WhatsApp and chatbot channels answer, collect details and propose an appointment; it is available to set up today, but it has not yet answered calls for a real clinic, so there is no measurement to quote. If out of 20 missed calls a month one in 20 ended in surgery, that would be EUR 3,000; it is an assumption, not a promise. The front desk that cannot keep up describes the problem; missed calls are its cost.
2. The quote that goes cold. A quote built on a measurement and sent the same day is easier to defend than an eyeballed estimate (see 3D scanning, the graft calculator and The quote that never closes). If you close one more consultation a quarter for that reason, you have already covered a EUR 500 monthly fee.
3. Staff time. The AI scribe proposes the record fields while the consultation happens and the doctor reviews them; the schedule with reminders cuts the work of confirming appointments by hand (scheduling and reminders). No invented figures: for every 5 minutes you save per consultation, 80 consultations a month is about 6.7 hours, and at a staff cost of EUR 40 an hour (an assumption) that is about EUR 270 a month. Time it with a stopwatch for two weeks before and after.
4. Paperwork that does not get lost. A single record avoids the searching that a scattered record costs; the calculation of that cost is in that article.

What cannot be promised today

• The AI scribe is in testing. It only records with the patient's consent, and until a lawyer has reviewed the consent text, in production it is rehearsed with test patients and demo clinics. Do not count its hours as savings until you use it with real patients.
• Quotes in the patient's language. The quote PDF is generated in Spanish today. The AI channels reply in the language of the incoming message, but a multilingual document is a pending improvement, with no date.
• Results. No software guarantees more surgeries. What changes is that every step (call, consultation, quote, follow-up) stops depending on one person's memory.

How to run your own numbers

1. Write down your surgeries per month and your average price.
2. For two weeks, log missed calls, note-taking time per consultation and unanswered quotes.
3. Decide what share you think is reasonable to recover, whether 5% or 20%.
4. Compare against the plan that fits you using the market analysis and the pricing page, which is the authority.

If the result is not clear, do not buy. If it is, request the demo and check each assumption against your own data.

How many extra surgeries does a clinic need to pay back the software?

At the example price of EUR 3,000, a EUR 1,000 monthly fee is covered by one extra surgery every 3 months or so, measured on revenue. With a smaller margin per surgery you need somewhat more; the formula is the fee divided by what each surgery leaves.

Are saved hours real money?

Only if those hours turn into something: more consultations handled, fewer overtime hours or fewer hires. If the saved time just evaporates, the saving is on paper but not in cash. That is why it is worth measuring before and after.
