Nurse
Signing on the tablet
Before the scan, the nurse opens the record's consent screen, sees what is missing and hands the tablet to the patient. Who collected the signature is recorded. If the patient isn't there, the nurse emails them the link.
Consent forms and signing
Each clinic has its own templates. The patient signs on the consultation tablet or from their portal, and an exact copy of the text they signed is kept with its fingerprint, their IP and their date.
Paper consent forms get lost, outdated versions get signed and, when there is a claim, it is hard to prove which text was signed and when.

Patient
The patient receives an email, goes into their portal, reads the full text and signs. They don't have to print anything or come back to the clinic for it.
Director
Faced with a claim, the director opens the signature, checks that the text has not changed since it was signed and sees when, from which IP and with which version. If the text changed afterward, the consent shows as outdated.
Good to know This is traceability with a fingerprint, IP and server date, not a qualified electronic signature and not one with a third-party timestamp. The patient's scanner in their app carries its own consent with a checkbox and signature.
